Health status of Burmese refugees in India

Burmese refugees in India are of poor families living in substandard condition with insufficient health care system in a foreign developing country which also ranks low. Many of them come from the remote areas of Burma which are underdeveloped and neglected by the governments for decades. The level of education and economy as well as health status are extremely low and all asylum seekers are prone to infectious diseases. When arrived in Delhi there are new facilities available but it is awfully insufficient.

As per mandate the UNHCR provides health assistance to the recognized ones. There are hundreds of unrecognized. Burmese refugees do not know how the government of India helps for their in health. There are a couple of NGOs which are partners of the UNHCR to assist Burmese refugees. However, recently there have been noisy complaints against those NGOs for corruption, discrimination, inefficiency and etc. The complaints by Burmese refugees could not be wrong because previously one health NGO of such had been disallowed.

Burmese refugees are encouraged to seek medical treatment from government hospitals and dispensaries. However they don’t get what they deserve. Professionally speaking apart from long queue, difficulties in communication, discrimination, the quality of health care is questionable. Recently a Burmese pregnant mother who is hepatitis B positive had unexplained and unfortunate labor process at a government hospital. It is learnt that unprofessional persons pushed with force the pregnant abdomen and the patient had to be transfused with 10 units of blood after labor and followed by post-delivery emergency laparotomy.

To promote health, Burmese community tried to set up own clinics. One of them is the Yamuna clinic opened since 2002. The data collected by Yamuna clinic reveals the actual health status of Burmese refugees taking shelter in Delhi.

The clinic is free for all Burmese refugees. It is run by a Burmese physician cum politician who request for donations and contribution from friends and sympathizers because most of the NGOs are not interested in running a clinic for refugees residing in a capital city. In the Q and A from a Burmese language radio, Daw Aung San Suu Kyi promised to support the clinic.

Nationality question is a matter of interest because Burma is country of many different ethnic nationalities. The doctor belongs to mainstream Burmese and the majority of refugees come from Chin, Kachin and Arakan States and also from Kuki area and Naga Hills. Among Chin refugees there are at least three different dialects. Therefore the doctor needs assistants who can speak different Chin languages.

The clinic assistants deserve appreciation because they are like all other refugees who have to manage for their living. But they volunteer for free service. Some of them have to spend from their pocket for local travel to come to the clinic. The clinic assistants have to come to clinic at any time if there is labor case. They are well-trained and good for community. They are the right ones who can provide health education among the community.

In 2010 there were total of 8,607 patients among them 62.3% was female patients and 37.7% was male patients. Majority of refugees suffer from infectious and communicable diseases. It is due to poor socio-economic living condition. Many girls, women and even men have anemia and vitamin deficiencies. Many of them have big families and sometimes the entire family members got infected. Seasonal illnesses such as flu, diarrhea, allergies, shin infections, viral hepatitis A, Dengue fever, and etc. are common.

Among the patients ante-natal care (ANC) is significantly common. Seeing that government hospitals are crowded and private clinic are expensive, Yamuna clinic opens for all labor cases. Since 2006 November, total of 207 babies have been delivered at the clinic. Vaccinations (BCG, Hepatitis B, DPT, OPV and ATT) for all newborn babies are also provided. But refugee women can’t get HPV vaccine for cervical cancer. The children have to pay for MMR, Typhoid and Measles vaccines from private centers. All are expensive.

Tuberculosis (TB) is not uncommon among Burmese refugees and the morbidity is rising. Refugees have to pay for laboratory tests and X-ray from outside and regular treatment is provided free by Yamuna clinic. Hepatitis B and C as well as HIV are also positive among the refugees. The extremely costly tests and treatments for those serious life threatening diseases are challenging. Some of them have no choice and let the disease take their course.

Burmese refugees have to work mostly in informal sector where there is no health insurance, no care for occupational health hazards and no compensation. There are accidental death reports.

Health education is not practically feasible for Burmese refugees because they have to work for many hours and have no time for education or political events. NGOs love workshops that do not fit for refugees. Though addiction and depression are common among the refugees psychological counseling is what Burmese refugees can’t get at all.

Most of Burmese refugees residing in Delhi have dream of third country resettlement. For that they have to undergo medical check-up before leaving. So for them healthy body is more important than English skill. For refugee children health is more important than education. There are a few outstanding refugees who have resettled in the third countries and send donations to the clinic. It can be translated that working and contribution for health is community based, different nationalities work for a common good, and a good practice of politics.

VATICAN CITY – The head of the U.N. AIDS agency told a Vatican conference Saturday that the pope had opened the door to greater dialogue with his groundbreaking comments on condoms and HIV prevention — even as Vatican officials stressed abstinence and marital fidelity as the best prevention.

Dr. Michel Sidibe, executive director of UNAIDS, was invited to speak to the conference on preventing HIV and caring for HIV-positive people, a significant event in and of itself, given that the Vatican usually only invites like-minded outsiders to its conferences and UNAIDS has not been like-minded on this issue at all.
UNAIDS holds that condoms are an "integral and essential" part of HIV prevention programs, which it says should also include education about delaying the start of sexual activity, limiting sexual partners and marital fidelity. The Catholic Church opposes condom use as part of its overall opposition to artificial contraception.
The Church does, however, play a crucial role in caring for HIV-positive people, particularly in Africa where some two-thirds of the world's 22 million infected people live. It runs hospitals and hospices, orphanages and clinics and has played a critical role in helping to de-stigmatize those with the virus and stress the need for changes in sexual behavior to stop its spread.
But the Church has long been accused of contributing to the AIDS crisis because of its opposition to condoms.
That was why Pope Benedict XVI made headlines last year when he said in the book "Light of the World" that a male prostitute who intends to use a condom might be taking a first step toward greater responsibility because he is looking out for the welfare of his partner.
"This is very important," Sidibe told the conference. "This has helped me to understand his position better and has opened up a new space for dialogue."
At the same time, however, the Vatican officials speaking at the conference either glossed over or made no reference whatsoever to Benedict's condom remarks — evidence of a certain "one step forward, two steps back" mentality that often characterizes developments in the Catholic Church.
Monsignor Silvano Tomasi, the Vatican's envoy to the Geneva-based U.N. agencies, cited several other Benedict quotes from the book, but not the condom comments. Monsignor Zygmunt Zimowski, head of the Pontifical Council for Health Care Workers, which hosted the meeting, didn't mention Benedict at all, citing instead Pope John Paul II about the "crisis of values" behind the AIDS crisis. Monsignor Jacques Suaudeau of the Vatican's bioethics advisory board briefly showed a slide with the remarks but didn't mention them.
The discrepancy reflects to some degree the way in which the pope's remarks were received. Progressives saw his comments as a justification of condom use in a break with church teaching; conservatives insisted he wasn't altering doctrine and that the opposition to condoms remained. After three attempts at clarification, the Vatican eventually issued a definitive ruling saying the pope hadn't changed church teaching.
Nevertheless, the impression left at least within the AIDS community was that he had made an opening — and Sidibe latched onto that Saturday.
Sidibe said previously the AIDS community and Catholic Church were "talking over" one another and often worked in opposition to one another in dealing with the AIDS crisis. But he said Benedict's words had opened a new possibility for working together, particularly in agitating for greater access to anti-retroviral treatments for the world's poorest patients.
"Yes, there are areas where we disagree and we must continue to listen, to reflect and to talk together about them. But there are many more areas where we share common cause," Sidibe said.
Increasing access to treatment has become an even greater rallying cry following the recently published results of a nine-nation study showing that HIV-positive patients who received early treatment were 96 percent less likely to spread the virus to their uninfected partners.
Sidibe called the research a "game-changer" in the fight against AIDS, particularly for couples where one person is HIV-positive.
Zimowski concurred, saying it even gives hope to such couples who want to have children — in other words, sex for procreation in keeping with church teaching.
That said, all 1,763 couples in the National Institutes of Health study, where one partner had HIV and the other didn't, were urged to use condoms and the study's authors stressed that condoms remain crucial for protection.
The Vatican's emphasis on the need for changes in sexual activity has been boosted by studies showing that at least in Africa, prevention programs focusing on condom promotion aren't working and that what works is male circumcision and reducing the number of sexual partners.
Yet Dr. Edward Green, former director of the AIDS research project at Harvard University, said there is little financial support for programs that advocate partner reduction, particularly among Western donors who uniformly insist on condom distribution as part of prevention efforts.
Green says he belongs to no particular church and bases his findings on empirical evidence, not morality.

NEW DELHI: It's time you cut that flab and shed those extra kilos. Particularly if you are a woman. It's just not about that extra spring in your step, it's also about staving off a lifethreatening situation if you land on the operation table. Doctors say, they are finding it extremely difficult to operate upon obese women.

Prominent surgeons in the city say they are taking recourse to minimal invasive techniques and natural access vaginal surgery to deal with the problem. And in a hitech leap, even robots are being used in surgeries.

In obese women, most of the space inside the abdomen is layered with fat. And this makes it difficult for the surgeon to cut through and reach the diseased portion. Moreover , post-operative management becomes extremely difficult . "In case of obese pregnant women, the growth of the foetus is stunted and the risk of miscarriage becomes high, said Dr Suneeta Mittal, head of gynaecology department , All India Institute of Medical Sciences ( AIIMS). Several other doctors echo the same opinion. Dr Urvashi P Jha, director, obstetrics and gynaecology, Fortis healthcare , said: "It becomes very difficult for the surgeon to reach the target. To remove a benign ovarian tumor, the scalpel has to cut through several inches of fat and this complicates the process."

She said a huge incision has to be made and it is difficult to carry out the procedure with fat stuffed inside the abdomen . It obliterates the view and position of the tumor, she said.

"To avoid this, we are using natural access route surgery for hysterectomy (removal of uterus) and removal of fibroids, benign tumours and cysts. In this technique, we access the diseased organ through the vagina, rather than make an incision in the body. The intestine and peritoneum , which are usually layered with fat, are avoided. There are less complications and no scars," said Jha.

Dr N K Pandey, eminent surgeon and chairman of Asian Institute of Medical Sciences, said most obese patients also suffer from diabetes and hypertension.

"In such patients, the risk of post-operative infection is high and healing of wounds takes a longer time. Obese patients are also at an increased risk of developing blood clots in lungs after surgery. This can be fatal. Also, we have to take precautions in administering anesthesia and constant monitoring of patients is required.

"Formation of clots can lead to pulmonary embolism, which is a life-threatening condition. So, oxygen levels have to be constantly monitored ," said Pandey.

At some centres in India and abroad, natural orifice technological endoscopic surgery is being experimented with.

Surgeons can operate on a patient without any incision by entering through the mouth and other natural routes.

"The uterus of a 58-yearold obese patient suffering from uterine cancer was removed with the help of a robot at our hospital recently. But with a restricted 2D vision and hand movement, laparoscopic surgeries are not possible on obese women," said Dr Sudhir Rawal, director, surgical oncology at Rajiv Gandhi Cancer Institute and hospital. He said, use robots in such cases because it provides 10 times more magnification and a three-dimensional view.

A group of scientists from Germany stated that the energy-saving bulbs release cancer causing chemicals when switched on and can result in severe troubles if located near to the head.

It is already known that the mercury with injurious effects is discharged at the time of bulb crocks up, now this newest research added one more aspect to the list of demerits that the cancerous chemicals are expelled when the energy- saving bulbs are turned on.
Besides this, the chemical discharge takes the form of stem with constitution of toxic partials including phenol, naphthalene and styrene.
The power of these toxic chemicals can be approximated high as were utilized by the Nazis to destroy lakhs of Holocaust victims.
Andreas Kirchner, an associate of the Federation of German Engineers, stated, "Electrical smog develops around these lamps. I therefore use them only very economically. They should not be used in unventilated areas and definitely not in the proximity of the head."
The materials utilized in the fabrication of energy-saving lamps are held responsible for the harmful effects and the country's Environmental Protection Agency is proposed to publish a warning in glare of publicity.

CHENNAI: The number of children dying due to immunization-related complications has more than tripled after the government closed down three public sector vaccine labs in January 2008. The government has no clue about the causes while children continue to die, the latest being that of five infants in Gujarat on Wednesday.

Details given by the Union ministry of health and family welfare in reply to an RTI query filed by Dr KV Babu of Payyanur show that 111 children died in 2008 due to "adverse effect after immunisation (AEFI)". AEFI is a general term that covers various reasons, including bad vaccine quality due to break in cold chain, contamination and complications due to pre-existing conditions of the child.

In 2009, the latest year for which the ministry has given details, 116 children died due to AEFI. In contrast, 32 children lost their lives to AEFI in 2007.

The government closed down its three labs — Central Research Institute in Kasauli, BCG Laboratory in Chennai and Pasteur Institute of India in Coonoor — in January 2008, citing the non-compliance of good manufacturing practices. To make up for the demand of 75 lakh vaccines doses of six kinds needed for its universal immunisation programme, the government has been proc-uring them from private firm.

The ministry has not found anything amiss. "In a country where 25 million children are born every year, it won't be proper to jump into conclusions. We are following the protocol of investigations," health secretary K Chandramouli told TOI.

Experts don't think so. "India is not following the Brighton Collaboration criteria, adopted by WHO to deal with AEFI deaths," said Jacob M Puliyel, head of paediatrics at Delhi's St Stephen's Hospital. The WHO protocol says that an AEFI death should be considered 'certain/very likely' if it occurs immediately after vaccination and if no other disease or drug is found as reason. "Here, we just test the vaccine and say there is nothing wrong," said Dr Puliyel.
Child health and immunisation deputy commissioner Ajay Khera said, "About 20 lakh children under 5 die every year. This converts to approximately 5,000 deaths every day, and hence a few deaths after immunisation cannot be linked to the vaccine.

The presence of a protein may help explain why some people are apple shaped and others pear shaped.

Researchers at the University of Edinburgh have pinpointed a protein that plays a part in how fat is stored in the body, potentially opening better ways to treat obesity.

Levels of the protein, known as 11BetaHSD1, tend to be higher in the presence of an unhealthy type of body fat which tends to be stored around the torso, typical of "apple shapes", the journal Diabetes reports.

Healthier fat, linked to lower levels of the protein, tends to be stored around the hips and is used more safely by the body as a source of energy, typical of people who are "pear shaped", according to an Edinburgh statement.

The study found that mice with the protein in their bodies were more likely to have unhealthy fat tissue after four weeks on a high fat diet, compared with mice without the protein.

Scientists are already looking at ways to make medicines that inhibit this protein, which is known to raise levels of hormones linked to obesity.

Nik Morton, of the University of Edinburgh's Centre for Cardiovascular Science, said: "This study opens up new avenues for research, and gives us a much better idea of why some fat in the body becomes unhealthy while other fat is safely stored for energy."

MYANMAR: Rights abuses lead to health "catastrophe" - report
BANGKOK, 7 March 2011 (IRIN)
Human rights abuses and counter-insurgency campaigns in Chin State, western Myanmar, are causing a "health catastrophe", says a new study, results which echo an earlier survey in the eastern state of Shan.

"The indirect health outcomes of the abuses likely dwarf the actual killings by the military leaders," said Richard Sollom, deputy director of the US-based NGO Physicians for Human Rights (PHR) and author of a recent medical study on Chin State.

Persecution of civilians has resulted in a "man-made health catastrophe" in ethnic areas, according to Vit Suwanvanichkij at the Johns Hopkins Bloomberg School of Public Health in the US.

In eastern Myanmar, the child mortality rate is almost double and maternal mortality triple the national average (122 per 1,000 live births and 380 per 100,000 live births, respectively, according to World Health Organization, WHO), which is related to the "epidemic" of human rights abuses committed against ethnic groups in that area, said Suwanvanichkij.

"Public health indicators in ethnic areas are similar to that of long-term war-torn countries like Rwanda and Sierra Leone," said Debbie Stothard, coordinator of the Bangkok-based Burma rights advocacy group, Altsean.

Nearly one-third of all households in Shan State in eastern Myanmar reported one or more human rights abuses from 2009 to 2010, according to a separate 2010 health survey, conducted by community health organizations.

"Consistently, the minority of deaths are directly from violence. Far more... lose their lives indirectly from the malign neglect of the Burmese military regime," said Suwanvanichkij.

Not only does the government invest the least worldwide in its healthcare system - less than 2 percent in 2009 according to the WHO - but rights groups say it also obstructs healthcare in conflict areas.

"The military actively prevents healthcare workers from doing their jobs by destroying healthcare facilities controlled by communities or insurgent groups," said David Scott Mathieson, Myanmar's senior researcher for Human Rights Watch.

Six out of 10 deaths among internally displaced persons in the east are from preventable illnesses, said Suwanvanichkij.

Babies and hunger

Shan State researchers found the occurrence of human rights abuses in a household increased the odds of an infant dying by up to 50 percent.

In Chin State (western Myanmar), almost 92 percent of households experience forced labour and reported the military stealing or destroying their family's food stock.

"With already amongst the worst food insecurity in the world, such predatory practices by the government can easily push families over the brink - worsening poverty, increasing malnutrition, and further barring access to healthcare," said Suwanvanichkij.

Forty-three percent of families in Chin State reported moderate to severe household hunger to PHR.

Families in Chin State whose food was destroyed or seized reported hunger levels more than six times higher than those left alone, according to PHR.

"If people are forced to give food to the military out of fear, have household crops destroyed or their food stores stolen and livestock stolen or killed, they are obviously going to be more likely to suffer from hunger," said Sollom.

Displacement and disease

Displaced families are about three times more likely to have acutely malnourished children, according to PHR. Almost half a million people are displaced in Myanmar, according to the Mae Tao Clinic in Mae Sot, Thailand.

"Being on the move in the rainy season without access to clean water leaves them vulnerable to malaria and diarrhoea - a major killer of children under five," said Stothard.

Local health workers continue to risk their lives in conflict zones to bring much-needed healthcare to the displaced, but as long as human rights violations continue, the health crisis will not improve, Stothard added.

'Cure@Home' unites the best practices in Allopathy, Ayurveda, Homeopathy and Yoga.

Mr. Medha Dhurndhar, Programme Coordinator, C-DAC which falls under the Central government's section of Information and Technology, said, "Cure@Home is designed as a promotive, preventive and curative health care solution to the common man."

The software applications cover diet and lifestyle principles, scrutiny and storage of records relating to individual structure and health grade, home therapies with holistic solutions for primary care and prevention of warning signs as well as illnesses.

Moreover, the software also has an encyclopedia comprising effigies, audio-video clippings and articles to boost health knowledge.

The newly formed software can be deployed on personal computers, laptop computers, touchscreen and can be utilized for practical use at home, medical institutions, infirmaries, public places plus educational institutions as a health care device.

Of the over 33 million HIV-AIDS cases reported worldwide every year, around one-third are adolescents of the age group 15-24 years, revealed a report by United Nations Children's Fund (UNICEF).

The State of the World's Children (SOWC) report 2011 was released by UNICEF in the capital Friday.

According to UNICEF's annual flagship report on the adolescents (10-19 years) in the world, the risk of HIV infection is considerably higher among adolescent females and young women than their male counterparts of the same age group.

"There is lack of comprehensive and correct knowledge related to HIV. In developing countries, the problem is even more serious," said Karin Hulshof, representative of UNICEF India.

According to the UNAIDS, there are about 7,400 new infections diagnosed every day, with about 40 percent of those being in the 15-24 age group.

Only 30 percent males and 19 percent females have correct knowledge about the disease, the report said.

The report recommendations called for greater involvement and investment by the government as India shared nearly 20 percent of the adolescent population in the world.

"India is home to more than 243 million adolescents of the total 1.2 billion adolescents across the globe. We have to consider that this strata is the future of the country, especially when we call India a young nation," said Naveen Jindal, MP and convenor of parliamentary forum on children.

The ministry of women and child development (MWCD) also announced the success of its pilot programme SABLA -- Rajiv Gandhi Scheme for Empowerment of Adolescent Girls -- aimed at addressing the problems of adolescent girls between 11 and 18 years.

The central government had doled out nearly Rs. 1,000 crore for the central scheme that is being implemented through Integrated Child Development Scheme (ICDS) projects and anganwadi centres.

"We plan to reach out over one crore girls in 200 identified districts after a baseline survey was conducted. Ministry will consider more such models if SABLA is successful," said D. K. Sikri, secretary, MWCD.

The report also said that with 81 million young people out of work globally in 2009, youth unemployment remains a concern in almost every country.

"In the year 2008, youth were nearly three times more likely to be unemployed in India," said a UNICEF official who was a part of the study.

More than seventy million adolescents of lower secondary age are currently out of school, and on a global level girls still lag behind boys in secondary school participation, the report mentioned.(IANS)

A group of Lambada tribal women sit huddled in Kannaram village of Medak district clutching prescriptions and plastic bags with multi-coloured pills. They pop these, they say, for relief from the various aches and pains they have been battling ever since they underwent the "operation".

Most women in this village of 125 households have had a hysterectomy - even those as young as 20, some even younger and in their teens - for complaints as routine and as easily curable as abdominal pain or white discharge.

Doctors in the rural pockets of the state, however, do not prescribe antibiotics to these hapless women. They prescribe complete removal of the uterus and ovaries, cutting short their reproductive lives and snapping their estrogen supply.

And it is not just the women of Kannaram who have hit an early menopause in Andhra Pradesh. A disturbing number of women in rural pockets from across the state have gone under the knife, surrendered their savings and losing their uteruses and ovaries to quacks who market hysterectomies as aggressively as salesmen sell water purifiers in cities.

These victims of rampant medical malpractice and government apathy now make for a startling statistic of the National Family Health Survey - the number of young menopausal women is the highest in Andhra Pradesh, standing at a staggering 31.4 per cent.

But what do doctors gain from the cruelty heaped on the women? Money. A hysterectomy can fetch anywhere between Rs 10,000 and Rs 50,000, depending on the facilities they showcase. On the other hand, treating a routine gynecological problem with pills wouldn't earn them a fraction of that.

The modus operandi used in the hysterectomy racket is simple. A woman, often illiterate and unaware, lands up at a private rural clinic complaining of, say, excessive bleeding or leucorrhoea (vaginal discharge) and she is prescribed a hysterectomy as the only way out. A senior doctor who conducts medical camps in rural parts of Andhra Pradesh says, "My routine question to women in villages now is, 'Do you have a uterus?'" The women are told that they would die if they don't stem the discharge or bleeding with a surgical process.

Manjula, one of the unfortunate lot, is not quite sure why she had to have her womb scalped. And it's affected her in more ways than one. At 28, she can pass off for a 40-year-old; she tires easily and often feels older than even her 45-year-old mother. Having undergone hysterectomy five years ago, the daily wage worker says she is unable to toil on the farms anymore. She spends on medicines the little that she earns."I have been spending Rs 500 a month on medicines and I visit the doctor every week. He tells me to eat healthy food and rest, but can I do that?'' she asks, knowing the answer only too well.

An ultrasound scan of the abdominal and pelvic area in the presence of this reporter confirmed the women's claim that they had undergone complete hysterectomy. Gynaecologists, at least the ones with ethics, maintain that total hysterectomy is rarely warranted these days. Whenever hysterectomy seems inevitable, an effort is made to salvage the healthy part of the ovary so that the woman's body continues to get some supply of estrogen, they add.

Clearly, such caution finds no place in the hysterectomy market of Andhra Pradesh, where scores of women undergo the procedure in makeshift operation theatres in roadside clinics. Some of these are two-room tenements, with one part serving as a clinic and the other as OT. Interactions with these women and studies on the subject have established that the procedures are more often than not performed by registered medical practitioners (RMPs) or city-based general surgeons who zip over to small town clinics over weekends. Village quacks provide the necessary reference work.

While this medical malpractice was highlighted five years ago by the Centre for Action Research and People's Development (CARPED), an NGO that works mainly in Medak district, a recent state-wide study undertaken by Andhra Pradesh Mahila Samatha Society (APMSS) shows most women who have undergone the procedure are suffering from health problems ranging from fatigue to osteoporosis.

The study, which was conducted across 225 villages in five districts, concluded that the issue was not restricted to a particular area but spread across the state."In none of the cases did the women undergo a thorough medical check-up before the operation and there was no counselling on the hormonal imbalances leading to early menopause and other related health conditions,'' the study stated. The reason why women opted for the procedure ranged from excessive bleeding to irregular menstrual cycle.

What has shocked health experts is this rampant practice remaining unnoticed. State health officials confirm that hysterectomies in AP are the highest in the country given the number of women involved, and they also admit that the government is insensitive to the plight of these women. Sources in the ministry of health note how hysterectomy is a growing racket involving quacks, private hospitals and RMPs.

Dr M Prakasamma of the Academy for Nursing Studies fishes out her 2002 study, which found that "more and more women of younger age group are observed to be undergoing hysterectomy''.

Eight years later, not only has the government remained seemingly oblivious to this health tragedy, it has also inadvertently pushed the trend further. The state health insurance scheme, Arogyasri, is being used to fund these operations. This scheme pays for the private treatment of those below the poverty line.

In fact, in 2008-2009 alone, around 11,000 women were hysterectomised and each procedure was funded by the state government. Sources say that in many of these cases, the age of the woman was between 25 and 35.Alleging that nursing homes in the districts were even fudging the date of birth, they say in at least 50 per cent of cases the woman was less than 40 years old.

Some health officials are now demanding that hysterectomies be scrapped from the list of procedures covered under Arogyasri, but their voices are not aggressive or persuasive enough. Arogyasri, anyway, is a recent reason for the rise in such cases. Veena Shatrugna, former deputy director of the National Institute of Nutrition, points out another unnerving factor driving the trend: sterilisations.

What happens is that sterilistations are done after the birth of the last child when a woman is in a pre-menopausal stage. Family planning procedures are often carried out in government camps (to meet targets) and women quite commonly develop infections that need treatment. Unscrupulous doctors are quick to suggest a completely unnecessary hysterectomy.

Shatrugna's analysis fits in with the facts and figures. The APMSS study on hysterectomy found that 82 per cent of the women who had undergone a hysterectomy had had a family planning operation prior to it.

Doctors believe that rural women develop infections because of poor menstrual hygiene."But you do not remove the uterus for that,'' says Dr A Vani, who heads the primary healthcare centre in Kannaram village. She admits that women prefer visiting RMPs to her centre because they see hysterectomy as a way out of dealing with periods.

Activists working on the issue squarely blame the government for its apathy."We have been highlighting the issue since 2005 and have met health officials too. But nothing has been done about it,'' says M Subhash Chandra, director, CARPED.

Dr PV Ramesh, principal secretary, health, agrees that the "the issue is serious'' and that the government is looking at launching a campaign to raise awareness among the masses. Until that happens, the number of menopausal women will keep piling up in Andhra Pradesh. And till then the stories of helpless rage will keep pouring in.

"I sold my one-acre plot and even my gold jewellery for my medication. But my abdomen still hurts,'' says Bhuli, who hit menopause at 30 when she underwent hysterectomy.

While others calculate the thousands they have lost on treatment, 25-year-old Maruni broods over the other incalculable loss. Mother of one, she underwent a hysterectomy at 22."I couldn't work on the fields anymore. So I was of no use to my husband who left me for another woman. And then I lost my only son,'' she says,even as the RMP she consulted continues to flourish in the village.

Healthcare experts say the hysterectomy racket has its roots in the privatisation boom in the medical sector back in the mid- '80s.Till star politician NT Rama Rao came to power, healthcare was state-driven in Andhra Pradesh.

"It is around this time that the practice of unwanted hysterectomies first started showing up in the state,'' notes Veena Shatrugna, former deputy director of National Institute of Nutrition. She says the number of such procedures have steadily risen over the last two decades.

In government hospitals, poor patients are never randomly advised hysterectomies because government doctors stand to gain nothing by doing so."But private hospitals are not accountable to anyone and people were willing to pay for the procedure,'' she says.
Private doctors have also managed to wean away the queues from public hospitals with promises of quicker and better care.

Soon, these doctors, both MBBS and RMPs, became the points of contact for private nursing homes and allegedly started getting a 30 per cent cut (from the cost of treatment) for each case referred. Worse, many of them started performing the procedure themselves.

"People can also be so naive. They believe that once the uterus is removed all their troubles would end,'' says Dr Choranur Ambuja, secretary (Hyderabad chapter), Indian Menopause Society.

Health activists now hope to spread greater awareness about medical malpractices in the villages."The only way to curb the practice is by improving awareness. And in the absence of government will, only responsible doctors can do it,'' says an activist.
